AGREEMENT TO TREAT

FORM 2 of 2

This agreement will be valid from the date below and will include all pets in The Client’s household.

I, The Client hereby agree to allow Pacific Pet Nurses (PPN) to treat, care and perform services requested upon my pet(s). 
I am aware that at any time Pacific Pet Nurses may cease treatment due to patient stress, aggression or potential injury to themselves or others. If this happens 50% of service rate is due. 
Pacific Pet Nurses may use restraint at the discretion of the LVT’s working with my pet. Forms of restraint used may be manual, muzzles, or towel restraint. 
I am aware that in the event of a traumatic injury to Pacific Pet Nurses caused by my pet, my pet’s information may be disclosed to healthcare officials.

What to Expect

Pacific Pet Nurses, PLLC is happy to offer at home services for your beloved pets! 
Performing treatments in the comfort of the patients home, or in our Fear Free friendly mobile unit; wherever the patient feels most comfortable!

Blood Pressure Appointments: Typically the patient feels most comfortable with the owner helping for these appointments. We recommend having the patient in a calm comfortable location (their favorite bed, your lap, etc.) for this procedure. With kitties we find it is helpful for them to be in this calm location with their owner prior to our arrival, and we can enter the home without knocking or ringing the doorbell. 

This procedure involves shaving a small spot on your pet for probe placement, placing a cuff and listening to the arterial flow. PPN techs wear headphones during this process to lessen "scary" noises to our patients. We also ask that cell phones are 4-5 feet away during these appointments to reduce electrical interference. 

If your pet typically receives sedation prior to their regular vet appointments, please discuss sedation protocol with your DVM for your pets in-home appointment.

Hours of Service

Appointment times are approximate, PPN will stay in contact with you for your specific arrival time the day of your appointment.
PPN does not have set hours of operation. Instead, we base planning our schedule off of DVM referral and patient needs, which does often fluctuate. Many of our clients like to book repeated appointments month(s) in advance, so we do offer appointments Tuesday's and Friday's on a set weekly basis for that purpose. Typically, appointments can be scheduled within 10 days of inquiry, with the exception of weekend appointments which are limited.
We do our very best to accommodate the needs of our patients in a timely fashion and we appreciate the understanding of those we serve!
